[tex]\sqrt{128x^{3} y^{4} }  = [tex]\sqrt{8*8*2*x*x*x*y*y*y*y}[/tex]

Take the perfect squares out the expression to simplify

[tex]8xy^2\sqrt{2x}[/tex]
